Abs Title: PHOSPHATE SUPPLEMENTATION IN CRITICAL LONG TERM MECHANICALLY VENTILATED PATIENTS INCREASES VENTILATOR FREE DAYS

Rationale: Critically ill patients have a high prevalence of hypophosphatemia because of the presence of multiple causal factors. Hypophosphatemia is associated with respiratory muscle dysfunction, potentially resulting in weaning problems.
Methods: Randomized controlled study – we included 152 critical patients over a period of 12 months, APACHE II over 22, mechanically ventilated for more than 7 days. Patients were divided in two groups - ARDS group (defined as PaO2/FiO2<200) - 68, and non ARDS group - 84.All patients received nutritional support started in the first 24 h from ICU admission, starting with enteral route and early parenteral supplementation. Patients from both groups were randomly assigned to receive or not phosphate supplementation .
ARDS group - control group(22) : no phosphates supplementation
- Study group (20): 20 ml Glicophos/day
Non ARDS group – control group (33): no phosphates supplementation
- Study group (33): 20 ml glicophos/day
All patients were ventilated with “lung protective” strategy, with every day sedation and ventilation trial of weaning.We evaluated : negative inspiratory forces, days of mechanical ventilation, rate of reintubation for respiratory distress and ICU length of stay.
Results:
Conclusion: Patients coming from boh groups who received parenteral phosphates supplementation had higher values for NIF, fewer days on ventilator support and smaller rate of reintubation.We didn’t obtain statistically significant data regarding length of ICU stay.
