Abs Title: NUTRITIONAL ESTATUS ON ADMISSION AND DISCHARGE IN A REGIONAL HOSPITAL

Rationale: Recent studies show that prevalence of hospital malnutrition in Spain ranges from 20 to 40%.Our aim is to determine the prevalence of malnutrition in a regional hospital and to analyze the relationship between nutritional status,hospital stay,complications and patient progress.

Methods: Observational and randomized study analyzing medical and surgical patients admitted to our hospital from January to May 2012.The MUST and MNA screening test according to age,were undergone on admission and a nutritional assessment was recorded both on admission and discharge.Variables:sex,age,diagnostic,weight,BMI,weightloss,risk or malnutrition (R/M),percentage of food intake,reasons food intake,length of stay,complications and mortality.

Results: 213 patients; 48.8% men; 51.2% women; mean age 72±15.2 [24-99] years.A 44.6% of malnutrition prevalence was found.Elderly patients showed more risk or malnutrition:10% young vs 58,2% older p<0,001.Neurological patients were more at risk or malnourished(59%), followed by respiratory and oncology patients both with 50% R/M.Those patients with an intake <50% showed a 63.6% R/M,whereas patients with an adequate intake showed 38%,p=0.001.The main reason for a decrease intake was anorexia (48%) or an inadequate diet prescription (39.3%).Globally there was a weight loss of 0.5kg,p=0.001 although R/M patients encounter 1kg loss,p=0.001.In well nourished patients the length of stay was 8 days vs 10,5 d. in malnourished p=0,01.These later suffered a 56,1% of infectious complications vs 43,9% of those well nourished,p=0,024.Mortality account in 7.4% of patients with R/M vs 2.5% of those without ,p= 0.098.

Conclusion: 1 of 2 patients is at risk of develop or suffer malnutrition in our hospital.The use of screening methods is thus essential to detect early risk or malnutrition and it would influence the outcome of patients, optimizing hospital resources.
