Rationale: OS-1® is an oral rehydration solution (ORS) based on the guidelines formulated by the US and EU. A controlled clinical study was conducted to evaluate the preventive effectiveness of dehydration with OS-1® in preoperative pediatric patients.
Methods: Forty pediatric patients undergoing minor surgery (age of 1 to 7) were allocated to 2 groups: a preoperative fasting group (PFG), and an ORS drinking group (ORSG). PFG (n=20) was managed by fasting from 9:00AM until anesthetic induction, ORSG (n=20) was instructed to consume only OS-1® as much as possible until anesthetic induction. And blood and urine sampling was performed after anesthetic induction.
Results: There was no significant difference in patient profile and main biochemical data. Preoperative fasting time was 4h45m±30m, and amount of drinking OS-1® was 223±117ml. However, in blood ketone body · FENa · urine specific gravity, there were remarkably significant differences (table 1), and so preoperative fasting management in children has been proved to cause dehydration state even for a short time.

Conclusion: OS-1®(PET bottle type) was released in about 10 years ago in Japan. Preoperative management with this ORS will be free from the infusion line in not only children but adult patients, and increase their preoperative QOL. There is also reports that this ORS was extremely effective for patients with dehydration in the refuge area of the great east Japan earthquake and tsunami disaster in the last year. And so effective utilization of ORS is expected in many dehydration aspects.
