Rationale: MNA-SF® is widely noticed and used in Japan. But there is a big difference in BMI in Japan and in Western countries.(70-79 men 25.3±3.2 women 24.4±4.4 in Sweden, 25.5±4.3, 28.5±5.4 in Italy, 70-74 men 21.93±3.30 women 21.84±3.70 in Japan) To adjust this difference, we changed cut-off points on BMI in MNA-SF.

Methods: One-hundred eighty nine individuals (hospitalized 91, Nursing home residents 98, female 59.3% mean age 84.3±9.6)were evaluated with modified- Subjective Global Assessment (well-nourished, slightly-malnourished, moderately- malnourished, severely-malnourished) and Mini Nutritional Assessment-Short Form (well- nourished, at risk, malnourished)

Results:  BMI 18.40±3.23, Calf circumference (CC) 25.95±4.28 ,MNA-SF 7.56±2.89. Hospital : BMI 17.24±2.76, CC 23.69±2.92 ,MNA-SF 6.25±2.49. Nursing home : BMI 19.47±3.26, CC 28.05±4.29 ,MNA-SF 8.76±2.69. Modified-SGA : well-nourished 59 (31.2%), slightly-malnourished 64 (35.8%),moderately-malnourished 53 (28.0%), severely-malnourished 13 (6.9%). MNA-SF well- nourished 15 (7.9%), at risk 76 (40.2%), malnourished 98 (51.9%). Correlation MNA-SF to modified-SGA 0.607、MNA-SF to CC 0.680. If we use cut-off points 17,18.5,20 instead of 19,21,23, modified-MNA-SF 8.24±2.92, well-nourished 47 (24.9%), at risk 61 (32.3%), malnourished 81 (42.9%). Correlation modified-MNA-SF to modified-SGA 0.621、modified-MNA-SF to CC 0.733

Conclusion: Modified-MNA-SF (changed cut-off points) is suitable to modified-SGA  and CC than original MNA-SF in our data.
