ENDOTHELIAL PROGENITOR CELLS, ASYMMETRIC DIMETHYLARGININE AND OBESITY AFTER RENAL TRANSPLANTATION: AN EFFECT OF EXERCISE TRAINING AND NUTRITION

Rationale: The aim of prospective randomized study was to asses the relationship between peripheral endothelial progenitor cells (EPC), plasma ADMA and fat in muscle mass examined by magnetic resonance spectroscopy(MRS) in transplant patients after early exercise training and supplement of keto amino acids.
Methods: For 6 months we monitored 40pts (M 22/ F18, 20 – 70 yrs, BMI 24.5- 30.4 kg/m2) after kidney transplantation (Gr I).. Intervention involved exercise regimen and supplement of keto amino acids.Control Gr II of 40 non-exercising transplant patients matched in age,BMI,HLA,,dialysis, cardiovascular disease and immunosuppressive regimen.
EPC count in blood and MRS were performed at month 1. and month 6.
Results: Differences were found in plasma (month 1. vs 6., t- test, ANOVA): EPC 1.250±760cells/ml vs 2.350cells/ml. ADMA 3.72 ±0.43 vs 2.10±0.24 mmol/L, adiponectin 13.11 ±5.0 vs 22.5±5.0 mmol/mL, all p < 0.01 in Gr I.
MRS confirmed reducing of extracellular muscle fat from abdominal and calf muscle area after exercise training ( p<0.01).This effect was not found in patients with body/fat weight gain.Renal function was stabilized (GFR 63±12mL/min). BMI was not significantly changed.
Conclusion: Early exercise training and supplement of keto amino acids were associated with increase of EPC and adiponectin, decrease of ADMA and reducing of fat in muscles in non-obese patients. This regimen may decrease the cardiovascular risk after Tx.
