EFFECTS OF BODY WEIGHT REDUCTION ON BODY FAT AND PLASMA LEPTIN IN OBESE PATIENTS WITH TYPE 1 DIABETES MELLITUS

Rationale: To analyze leptin metabolism in obese patients with type 1 diabetes mellitus (T1DM) we studied the influence of seven days of fasting and 21 days of low-calorie diet (LCD) on adipocytokine leptin and body fat.

Methods: The plasma leptin and free fatty acids (FFA) concentrations were measured in 14 obese patients with T1DM (42,6±9,4 years, BMI 32,4±2,1 kg m-2) and 13 non-obese controls with T1DM (36,9±13,9 years, BMI 22,6±2,1 kg m-2) before fasting, immediately after 7 days of fasting, as well as after 21 days of LCD. Controls were studied only after overnight fasting. Free fatty acids were analyzed with a FFA-HR kit (Wako chemicals GmbH). Plasma leptin was analyzed with a Bio-Rad Luminex system (Millepore, Billerica). Bioimpedance spectroscopy technique was used for determination of the body composition (BCM, Fresenius Medical Care). Statistical evaluation used means ± SD and One Way ANOVA.

Results: Obese T1DM patients lost 6,1±1,1 kg during fasting. There was a significant decrease in   fat tissue index measured 21 days after LCD (from 16,6± 2,57 to 14,9± 2,67  kg m-2, P< 0,05). Plasma FFA concentrations in obese T1DM patients before fasting were significantly higher than in controls (0,51±0,27 vs 0,25±0,17 mmol/l, P< 0,05) and increased significantly after fasting (from 0,51±0,27 to 1,03±0,42 mmol/l , P< 0,05). Plasma leptin concentration in obese T1DM decreased significantly after fasting  (from 16,5±17,0  to 5,79±6,40 ng/ml, P< 0,05) and after 21 days of LCD remained significantly higher than in controls (13,34±9,40 vs 4,29±4,55 ng/ml, P< 0,05).

Conclusion: Increased plasma FFA concentration marked insulin resistance in obese patients with T1DM. Fasting lowers leptin levels. Increased plasma leptin concentration after LCD can reflect improved hormonal signaling of the amount of fat tissue.
