EFFECT OF PROTEIN INTAKE ON NITROGEN LOSSES AND NITROGEN BALANCE IN PATIENTS WITH AUTOLOGOUS HAEMATOPOIETIC STEM CELL TRANSPLANTATION

Rationale: Effect of protein intake (PI) on nitrogen losses (NL) and nitrogen balance (NB) are poorly studied in autoimmune and oncohematologic patients with autologous haematopoietic stem cell transplantation (HSCT). The goal of the study was to define PI influence on NL and NB values in HSCT patients.

Methods: Autoimmune (n=129) and oncohematologic (n=82) patients with HSCT (Gr.A and Gr.O): age 34.2 ±11.4 ys. PI (g/kg), NL (g/24h) and NB (g/24h) were registered at 4 time-points. NL was measured as urinary urea nitrogen. PI interval was divided into 3 parts: <0.8; 0.8-1.6; ³1.6 g/kg. Mean values of body weight (BW), NL, NB, NL and NB per kg were calculated at each time-point and PI section. Statistics included ANOVA.

Results: In Gr.A NL increased from pre-HSCT to D0 but decreased at D+7 and D+14 for any PI while NL/kg increased for entire period of monitoring (for example, for PI³1.6: NL pre-HSCT 12.8, D0 26.5, D+7 23.2, D+14 19.5 g/24h while NL/kg pre-HSCT 0.22, D0 0.23, D+7 0.32, D+14 0.38 g/kg/24h). BW decreased for entire period of study (PI³1.6: 57.8 pre-HSCT, D0 61.8, D+7 54.6, D+14 53.0 kg). The same trend was observed in Gr.O. Relation between PI and NL was completely opposite to those between PI and NB for each time-point in both groups. While NL increased with PI increment, NB approached to zero (D+14 NL:  at PI<0.8 NL=15.8, at PI 0.8 to 1.6 NL=18.9, at  PI³1.6 NL=19.5 while NB at this PI levels were -11.4, -7.4 and -3.2 respectively).

Conclusion: UUN per kg should be used for correct NL monitoring to avoid errors due to decreasing body weight in HSCT patients. Higher PI causes normalization of NB despite parallel NL increasing.
