Achieving Excellence Together, now in its 18th year, is the preeminent event for Ontario’s home and community care sector. This free conference for health care leaders, professionals, providers, patients and caregivers demonstrates and promotes excellence and innovation in patient-centered home and community care.

The theme of this year’s conference is Transforming, Engaging, Integrating.

By attending Achieving Excellence Together 2017, you will participate in and contribute to transforming, engaging and integrating the home and community care sector through your commitment to knowledge-sharing, excellence, and continuous improvement.

Achieving Excellence Together conference sessions provide an opportunity to expand your knowledge and connect with colleagues in home and community care on topics such as community-based care models, big data analytics, digital health systems, and evidence-based practices.

All registration fees have been waived for the 2017 conference.

Once you have completed the registration form, you will receive an email confirmation with a link to the online conference program. There you can select the sessions you plan to attend.

Please note that sessions are not ticketed, and seating is not reserved.

Achieving Excellence Together is a conference hosted by Health Shared Services Ontario (HSSOntario), an agency of the Government of Ontario. HSSOntario is a shared services organization that supports Ontario's 14 Local Health Integration Networks in meeting the health care needs of their local communities.
